Dog Licenses / Park Permits 

State and municipal law (City of Middleton Code - Chapter 7) requires that all dogs five months and older be licensed with the city each calendar year. If you are licensing your dog for the first time or you have recently vaccinated your dog, a rabies certificate must be presented at the time of licensing. The city is proud to offer low-cost fees for licensing as listed below.

Licensing your dog is a requirement of the city. If your pet were to stray, the tag is a valuable resource to help bring them back home quickly. Cats do not need to be licensed.

Dog & Cat Limit: 16.05(17) No person shall harbor, shelter, or possess more than five dogs or cats or any combination thereof upon a single premise,

Dane County Dog Park Permits are also available. They are good at all Dane County Dog Parks for the calendar year.

Cost Per Calendar Year 


  • Dog Licenses: $30 for altered dogs; $60 for not altered dogs 
  • Annual Dog Park Permit: $46
  • Daily Dog Park Permits: $8 per dog
  • Senior/Disabled/Military Dog Park Permit (65+): $23

When applying online you will need to:
  • Complete and submit the Dog License Application and Payment Form
  • Upload a rabies certificate for each dog
  • Once you submit your license application,  you will enter your online payment method.  Credit card  (fees apply) or EFT (no fee).
